T2D Drugs and Reduced Risk of Kidney Stones

The Endocrine Society Annual Meeting and an article published online in the Journal of Clinical Endocrinology & Metabolism found that the T2D patients receiving empagliflozin, an SGLT 2i were 40% less likely to develop kidney stones in comparison to control patients receiving a placebo. The researcher i.e., Dr. Priyadarshini Balasubramanian, MD, who presented the poster at ENDO 2022 claimed that her team came to the conclusion through a retrospective, post hoc analysis where urolithiasis (a stone in the urinary tract) and nephrolithiasis (a kidney stone) were adverse events. 

Earlier, Kasper B. Kristensen, MD, Ph.D., and colleagues found that SGLT 2i increased the urinary glucose excretion, thus reducing the risk of stones by reducing the concentration of the lithogenic substance in urine. Based on the data collected from Danish Health registries, they found that the hazard ratio of 0.51 for nephrolithiasis and 0.68 for recurrent nephrolithiasis for patients taking SGLT2 inhibitors versus GLP-1 agonists.

EMPA-REG OUTCOMES TRIAL showed an incidence rate ratio of 0.64, which was in favor of empagliflozin. The rate was calculated based on the data collected from the older subjects of the trial, who had the longest follow-up period of 2.6 years. Meanwhile, when the analysis was restricted to new kidney stones, the incidence rate ratio was found to be 0.65, again in favor of empagliflozin.
